35  Ventricular Tachyarrhythmias

Note

Sustained VTach lasts ≥ 30 seconds, whereas non-sustained VTach lasts < 30 seconds.

35.1 Stable VT/VF

• Drug of choice: Amiodarone 150 mg over 10 minutes
  • repeat PRN if VT recurs
  • Maintenance infusion: Amio 1 mg/min for 6 hrs

35.2 Unstable VT/VF

• Shock
• Drug of choice: Amiodarone 300mg IV/IO push
  • If no cardioversion → Amio 150 mg IV/IO push
  • After conversion: Amiodarone 1 mg/min x 6 hrs, then 0.5 mg/min x 18 hrs
